Orientation for Full-Time Staff
Please access the following forms which you will need to print and complete. Once forms are completed please schedule a time with the Human Resources Director, 270-901-1115, to turn in completed paperwork.
Keep in mind that you should not begin employment until your paperwork has been submitted to Sherri Forester, HR Director, and the appropriate documentation attached. As a reminder please bring your supporting documentation for the I-9 and a voided check.

Use this check sheet to ensure that all required forms are completed prior to scheduling appointment with HR Director, Sherri Forester.
- Background Check Form
In accordance with KRS Chapter 164.281 as amended by House Bill 3, Section 19) KCTCS shall conduct background checks on all initial hires prior to their employment for all KCTCS positions. For purposes of this KCTCS Human Resources Procedure in compliance with KRS Chapter 164.281 (House Bill 3), the term “initial hires” is set forth as “those individuals who have not worked for KCTCS within the 365 days prior to their hiring/rehiring at KCTCS.” Therefore, for former employees who are to be rehired within one year of their previous date of separation from employment at KCTCS, a criminal background check does not have to be conducted.
- Personal Data Sheet
The I-9 has been created by the Immigration and Naturalization Service (INS) to verify that an individual is indeed eligible to work in the U.S. Therefore, all employers are required by the government to obtain the information on this form before hiring an individual.
- W-4
The W-4 form allows your employer to withhold the correct amount of Federal income tax from your salary.
- K-4
The K-4 Employee's Withholding Allowance Certificate is a legal form that shows your withholding allowances for the state of Kentucky and is required by the state for tax purposes.
Direct deposit is mandatory. Direct deposit means that your paycheck is automatically deposited into your bank account. You need to fill out this form so that the College is authorized and has the correct information to make sure your paycheck can be deposited promptly on payday.
Please complete the on-line sexual harassment training. The training will take approximately 30 minutes.
Payday is normally the 15th and 30th day of the month. Employees are paid on an arrears basis. (Example: Time worked the 1st through the 15th are paid on the 30th of that month; time worked the 16 through the end of the month are paid on the next month.)

Account Access Information
Employee self service in Peoplesoft is now available to all employees. Employee self service allows Faculty and Staff the ability to see selected Peoplsoft Human Resources and Payroll data online from any PC via the internet.
